This tool provides a list of physicians, nurse practitioners, and physician assistants who must be validly licensed (as required by state or federal law) and attest to meeting the following criteria as a BRIXADI Provider: (A) experienced in treating moderate to severe OUD; (B) have prescribed or administered BRIXADI at least twice in the previous year; (C) accepting new patients; (D) able to refer patients to other Healthcare Providers for administration of MOUD treatment, including BRIXADI and/or the provision of psychosocial services; and (E) have agreed to be included in the Braeburn Find a BRIXADI Treatment Provider tool.

The tool also provides a list of administration-only pharmacies that have the required legal permissions and are contracted with Braeburn to administer BRIXADI under the direction of a prescriber.

• Participating HCPs do not pay a fee to be listed in the tool, and their participation does not impose any requirements on the manner in which they treat or provide services to patients.
• Search results are based on the geographical information entered, sorted from closest to farthest.
• Some of the participating HCPs may be consultants to Braeburn and receive payment for services. Some may have other financial relationships with Braeburn as recipients of research grants or other similar payments, or they may have an ownership interest in Braeburn.
• HCPs may request at any time to be removed from the tool, and Braeburn reserves the right to remove HCPs from the tool.

Braeburn does not warrant, whether expressed or implied, that the tool is accurate or complete. Braeburn does not recommend or endorse any HCP included in the tool for any purpose, nor is the tool intended to verify a specific HCP’s credentials or qualifications. It is your responsibility to select an HCP appropriate for your needs, and you assume full responsibility for your interactions with any HCP that you contact through the tool. Braeburn is not responsible for the medical care or advice given by any participating HCP you choose to contact.
